The $4T surgery market claim hides where healthcare pain actually lives

·Commentary on CB Insights

Field service scheduling is broken. Everyone knows it, but Nick Damiano at Andromeda Surgical recently put a number to a different kind of problem: the $4T global surgical procedures market. It's a huge claim, and it got me thinking about where the real pain in healthcare actually is.

Damiano is building autonomous surgery to make every surgeon operate closer to the level of the world expert. It's a compelling vision. But when I look at the data we track on PainSignal, the picture gets more interesting. We're tracking 1,075 problems in healthcare with an average severity of 4.8 out of 5. That's not a typo. Healthcare is drowning in high-severity problems.

The kicker? Of the top 15 healthcare problems in our dataset, 13 are related to nursing or clinical documentation. Not surgery. Not autonomous robots. Documentation and nursing workflows. That's where the daily grind is, and that's where builders might find the most underserved opportunities.

Don't get me wrong. Surgery is a massive market, and improving surgical outcomes matters. But the $4T figure is hard to verify, and it likely includes broader healthcare spending rather than just surgical procedures. Meanwhile, we have 539 app ideas in healthcare on PainSignal, and only a small fraction are specific to surgery. The majority target general care delivery, documentation, and coordination.

Take ScrubIn: OR Onboarding & Simulation Trainer. It's one of the few surgery-related opportunities we track, and it's not about autonomous robots. It's about training and onboarding in the OR—a very human problem. On the other hand, problems like NurseAssign EquiLoad and MedSafe Flow highlight workload management and medication safety, which affect every patient, not just those in surgery.

I'm not saying autonomous surgery is a bad bet. But if you're an indie hacker or an investor looking for high-severity, high-willingness-to-pay problems, the data suggests you should look beyond the operating room. The nurses and clinicians wrestling with documentation every day are screaming for help. That's where the volume of pain is, and that's where a lean team can build something that gets adopted fast.

Andromeda Surgical is tackling a hard, important problem. But for every problem in surgery, there are dozens in nursing documentation, medication safety, and care coordination with severity scores of 5/5. The healthcare industry page on PainSignal shows the breadth of these issues. Builders who focus there might find a faster path to impact and revenue.

So yes, autonomous surgery could be game-changing for the few who need it. But the real goldmine in healthcare might be boring, unsexy documentation software. And that's exactly the kind of opportunity I love.
